Procedures on how to apply for PSAI membership:

1. Download and complete the membership form below:
   • Individual Membership Form
   • Institutional Membership Form
   
   
2. Pay the corresponding 2025 membership fee. Take a screenshot of your payment as supporting evidence.
   
   • Individual membership fee = Php 900.00
   • Institutional membership fee = Php 10,000.00
   
   
3. Submit the accomplished application form (preferably in PDF format, bearing your signature and photo) through this online portal: PSAI Membership Payment Form, along with proof of payment for the membership fee. (Suggested filename: "PSAI_membership_form_individual - Complete Name.pdf")
   
   
4. The PSAI Membership Committee shall evaluate your membership application and recommend for approval to the PSAI Board of Directors.
   
   
5. The PSAI Secretariat shall notify you once your application is approved.
